Please note:

• Your first appointment will consist of an initial evaluation. In office and Virtual appointments are available depending on the day of week and time

• Kristi does not accept next day appointment requests. This would include requests on Friday for a Monday appointment.

• After hours and Saturday appointments are offered by Kristi on an individual basis. New patients are unable to book after hours or Saturday appointments for their initial evaluation.

• Any appointment requested via the website is tentative until confirmed by the office.

• Once your first appointment has been confirmed you will be sent a link for our intake paperwork, please complete this upon receiving it. If intake is not completed at least 24 hours prior your appointment is subject to cancellation.

• Please contact your insurance company prior to your appointment if you are not familiar with your copay or coinsurance and whether a deductible must be met.

• Kristi’s self pay rate per session is $160.

• We accept most plans from the following insurance carriers:

  • Aetna

  • Blue Cross Blue Shield/Anthem

  • Cigna

  • Humana

  • United Healthcare

  • If your insurance is managed by a separate company and uses one of the above carriers networks please check with the office to ensure we are able to accept your plan.

Reminder - Kristi does not accept appointment requests online with less than two business days notice, this would include requests made on Friday for a Monday appointment. This allows her to finalize her schedule and prepare for her appointments appropriately. If the request is for a life threatening emergency please dial 911 or go the the nearest emergency room.
